Physio in Corsham is committed to ensuring your privacy and keeping your personal information protected.  When you use this physiotherapy service, you agree to your personal information being used by the physiotherapist, this is in order to provide the service to you.   Any information held about you will only be used for the purposes you have agreed to.  General statistics about customers may be used but will not include any information that identifies you.   If you believe that the information that is held about you is incorrect or incomplete, please let Izzy Tsalikis know as soon as possible.  This privacy policy explains how your information is collected and used.

Collection of your information:

Via email, letter, telephone calls, text communication, face to face meetings and information shared from other health professionals with your permission.   Through communication with Izzy Tsalikis via any of these methods, the content of the communication may be kept for the business records and your health records.

Sharing of your information:

Your personal information will not be sold, distributed or leased to third parties.  Your information will never be released to other customers or business partners, or to any other person or organisation for marketing purposes unless you have given your permission to do so.

 

How you can access your information:

You have a right to see a copy of the personal information that is held at Physio in Corsham under the GDPR.

 

How long is your information kept for:

Data will be collected throughout the duration of your physiotherapy intervention.  Following this the information will be kept for a further 7 years, unless at this time it is deemed necessary to retain it for a longer period.
